Developed by psychotherapist Arnold Mindell, it is an extension of Process Oriented Psychology (Process Work). It is based on observations by Arnold and Amy Mindell and colleagues (see books below) and others (Owen 2006, 2025) that at least some patients who appear non-communicative according to standard neuropsychiatric diagnostic criteria (Posner 2007) still experience their surroundings and are capable of communicating through subtle, often barely detectable nonverbal signals.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Foundations of Coma Work<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Consciousness is fundamental to our understanding of the world and is the only aspect of the universe that cannot be an illusion. However, the relationship between consciousness and the material world\u2014the external objects and events, and our experiences of them (the mind-body problem)\u2014remains unresolved. We lack a clear understanding of how the mind or consciousness arises from unconscious neuronal activity in the brain. While we can describe and observe correlations between specific brain functions and subjective experiences, the underlying cause of consciousness remains unknown.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Until recently, most scientists assumed that consciousness is a property of neural networks that emerges at a certain level of complexity. They posited that matter is more fundamental in the universe and that neurons are a prerequisite for consciousness. Yet, the intrinsic nature of matter appears increasingly fluid and intangible. The deeper we investigate it, the less certain we become about its properties: Is it a particle, a wave, a quantum, a quale, a form of field\u2014or something else entirely? Furthermore, because it depends on an observer, matter loses its independence from the mind. We can no longer clearly define what matter is; it cannot be considered independent of consciousness. Thus, a theory of mind that depends on matter, which in turn depends on an observer&#8217;s mind, is difficult to reconcile with a staunch materialist perspective.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients in deeply withdrawn, uncommunicative or comatose states have traditionally been considered by mainstream medicine to be victims of pathological processes that curtail normal cognitive and communicative functioning. Modern research, however, suggests that patients in these so-called persistent vegetative states may display &#8220;islands&#8221; of consciousness, minimal consciousness, or covert consciousness. Still, the range of comatose and vegetative states described by medicine is thought to be without intrinsic meaning, and the experiences of their victims without significance.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Coma Work recognizes individuals in deep, altered states of consciousness\u2014no matter how unresponsive they may appear to outside observers\u2014not as empty bodies but as inhabited ones. It interprets minimal signals such as changes in breathing patterns, skin color, slight movements, and any sounds or utterances as potential avenues for communication. By interacting with patients and amplifying their residual ability to perceive, process, and communicate, Coma Work builds relationships. This engagement encourages patients to become active participants in their recovery.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Since the ongoing general perception, despite new research, is that patients are incapable of understanding, thinking about, or communicating about their own condition, this view precludes their participation in their own care. Coma Work offers a new approach and sensitivity that addresses the whole being of patients in states where they cannot conventionally communicate about their inner experiences.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Patient Communication Channels<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Coma work begins with the belief that comatose patients, no matter how minimally, are capable of perceiving and relating to both outer and inner experiences. Therefore, the coma worker&#8217;s first task is to identify the communication channels available to the patient and then utilize them to connect with the patient&#8217;s experience. These channels can be identified by observing subtle, sometimes minute, signals such as movement, eye movement, facial expressions, and vocalizations. The coma worker then attempts to interact with the patient by engaging with and amplifying these signals.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Throughout this interaction, the coma worker is guided by patient feedback. For example, if the coma worker joins the patient in vocalizations, perhaps adding a bit of extra modulation, the patient may respond by altering their own vocalization. Additionally, the coma worker may try to establish a &#8220;binary&#8221; communication link, inviting the patient to use available movements, such as an eyelid or finger movement, to answer &#8220;yes&#8221; or &#8220;no&#8221; to questions.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Coma Work skills and sensitivities are beneficial not only for patients in so-called comatose and persistent vegetative states but also for those in altered states of consciousness due to dementia and near-death experiences. Medical progress has enabled us to sustain people&#8217;s bodies; however, we still lack the knowledge to understand consciousness or truly fathom what individuals experience during these life-altering processes when they are unable to communicate verbally and coherently.
